It is a complex experience involving the interaction of physical, emotional, and cognitive factors, activating not only the nervous system and the body's tissues but also the individual's mental state, thus reflecting its multifactorial nature (Melzack &amp; Wall, 1965).<\/p>\n<p data-start=\"373\" data-end=\"727\">In the integrative approach, pain is understood as a form of communication from the body. Each painful sensation conveys a message that needs to be interpreted, not simply silenced. The goal is not just to alleviate symptoms, but to perceive what the body is trying to express and restore functional balance in all its dimensions.<\/p>\n<h4 data-start=\"729\" data-end=\"765\">Pain as a protective mechanism<\/h4>\n<p data-start=\"767\" data-end=\"1112\">Pain is a natural defence mechanism. When the body detects a stimulus that could cause damage, such as inflammation, trauma, or improper movement, specialised receptors are activated and send signals to the nervous system. These signals are processed by the brain, which interprets them as pain, triggering a protective response (Woolf, 2010).<\/p>\n<p data-start=\"1114\" data-end=\"1438\">Under normal circumstances, this response is beneficial and adaptive, helping to prevent further damage and promoting tissue healing. However, when pain persists for weeks or months, even after tissue recovery, the nervous system can remain \u201con high alert,\u201d leading to chronic pain (Kosek et al., 2016).<\/p>\n<p data-start=\"1440\" data-end=\"1688\">In the integrative approach, the assessment is not limited to the physical origin of the pain, but also to its neurological, autonomic and emotional components. This global understanding allows for the construction of a more effective and personalised therapeutic plan.<\/p>\n<h4 data-start=\"1690\" data-end=\"1762\">When the nervous system becomes overactive: central sensitisation<\/h4>\n<p data-start=\"1764\" data-end=\"2053\">In some cases, the nervous system can react in an exaggerated way to stimuli that would not normally cause pain. This phenomenon is known as central sensitisation, an alteration in the way the brain and spinal cord process pain information (Latremoliere &amp; Woolf, 2009).<\/p>\n<p data-start=\"2055\" data-end=\"2080\">At this stage, the body can:<\/p>\n<ul>\n<li>Experiencing pain with light stimuli (allodynia);<\/li>\n<li>To amplify an existing pain (hyperalgesia).<\/li>\n<\/ul>\n<p data-start=\"2180\" data-end=\"2401\">Even without visible injuries, pain can persist due to neuroplasticity mechanisms. Treatment should therefore go beyond the area where the pain is felt, making it essential to retrain the nervous system (T\u00f6lle et al., 2009).<\/p>\n<p data-start=\"2403\" data-end=\"2582\">Through approaches such as Osteopathy, Physiotherapy, Clinical Pilates and therapeutic exercise, the intervention aims to desensitise the nervous system and restore body control.<\/p>\n<h4 data-start=\"2584\" data-end=\"2648\">A neuromatrix: the relationship between the brain and pain emotions<\/h4>\n<p data-start=\"2650\" data-end=\"2919\">The neuromatrix pain model, proposed by neuroscientist Ronald Melzack, suggests that the brain doesn't just \u201creceive\u201d pain, but interprets it. This interpretation is influenced by factors such as memory, beliefs, <em>stress<\/em> and the individual's emotional state (Melzack, 1999).<\/p>\n<p data-start=\"2921\" data-end=\"3361\">Pain, therefore, is a personal and multifactorial experience. In Integrative Osteopathy, we approach pain with a biopsychosocial perspective, treating not only the physical body (muscles, joints, viscera, craniosacral system), but also stimulating autonomic regulation, which is essential for re-establishing balance between the sympathetic and parasympathetic systems, often deregulated in cases of persistent pain (Jones et al., 2017).<\/p>\n<h4 data-start=\"3363\" data-end=\"3422\">Nociplastic pain: when the pain becomes self-sustaining<\/h4>\n<p data-start=\"3424\" data-end=\"3650\">Nociplastic pain occurs when the nervous system amplifies the perception of pain, without any visible injury. This type of pain is common in conditions such as fibromyalgia, myofascial syndrome, or chronic lower back pain (Clauw, 2014).<\/p>\n<p data-start=\"3652\" data-end=\"3716\">In these cases, effective treatment requires a combined approach:<\/p>\n<p data-start=\"3718\" data-end=\"4069\">\u2022 Deep myofascial release, to balance muscle and tissue tone;<br data-start=\"3797\" data-end=\"3800\" \/>\u2022 Joint and visceral mobilisation, to restore the body's overall mobility;<br data-start=\"3880\" data-end=\"3883\" \/>\u2022 Cranial and autonomic techniques, to regulate the nervous system;<br data-start=\"3950\" data-end=\"3953\" \/>\u2022 Therapeutic exercise and conscious breathing techniques, to consolidate balance and reduce sensitivity.<\/p>\n<p data-start=\"4071\" data-end=\"4176\">The aim is to re-educate the body and brain, promoting the re-establishment of safe, pain-free movement.<\/p>\n<h4 data-start=\"4178\" data-end=\"4240\">The integrative approach: rebalancing the body as a whole<\/h4>\n<p data-start=\"4242\" data-end=\"4380\">In integrative practice, the body is understood as a functional unit. Each therapeutic plan is personalised, acting on three levels:<\/p>\n<ul>\n<li data-start=\"4382\" data-end=\"4593\"><span style=\"text-decoration: underline;\">Structural<\/span>Joints, fascia, muscle balance;<\/li>\n<li data-start=\"4382\" data-end=\"4593\"><span style=\"text-decoration: underline;\">Functional<\/span> motor coordination, breathing patterns, posture;<\/li>\n<li data-start=\"4382\" data-end=\"4593\"><span style=\"text-decoration: underline;\">Neuro-emotional<\/span> <em>stress,<\/em> Stress, mental load and autonomic variability.<\/li>\n<\/ul>\n<p data-start=\"4595\" data-end=\"4765\">The combination of Osteopathy, specialised Physiotherapy, Clinical Pilates, and self-regulation strategies aims to reduce sensitisation and restore patient autonomy.<\/p>\n<h4 data-start=\"4767\" data-end=\"4809\">Pain as an opportunity for regeneration<\/h4>\n<p data-start=\"4811\" data-end=\"5097\">When pain is understood more deeply, it ceases to be a mere obstacle. Instead, it can be seen as an opportunity for change and evolution. Pain becomes a turning point, a signal that invites us to reconnect with our bodies and promote natural balance.<\/p>\n<p data-start=\"5099\" data-end=\"5442\">In an integrative approach, treating pain doesn't just mean relieving symptoms. It involves re-educating the nervous system so that the body learns to cope more effectively with pain.
