As the years pass, the body undergoes changes that can affect muscle strength, mobility, balance, cardiovascular health and some cognitive functions. However, ageing does not necessarily mean losing quality of life or independence.

Although factors such as genetics play an important role, scientific evidence demonstrates that lifestyle significantly influences how we age. Among the various health-promoting habits, the regular practice of physical exercise stands out as one of the most effective interventions for preserving functional capacity, reducing the risk of chronic diseases and promoting greater longevity with quality of life.

The World Health Organization identifies physical inactivity as one of the main modifiable risk factors for chronic diseases and premature mortality. In contrast, physically active people are less likely to develop cardiovascular diseases, type 2 diabetes, certain types of cancer, osteoporosis, depression and cognitive decline.

The good news is that it is never too late to start. Studies show that even people who begin an exercise programme after the age of 60, 70 or even 80 can achieve significant improvements in muscle strength, balance, cardiorespiratory fitness and independence in carrying out daily activities.

What happens to our body during ageing?

Ageing results from the interaction between genetic, environmental and behavioural factors. As age advances, there is a progressive decrease in the regenerative capacity of tissues and the functional reserve of the various organs and systems. Although these changes are part of physiological ageing, their speed and intensity vary considerably between individuals.

Among the main changes associated with ageing are:

Decreased muscle mass and strength

Reduction in bone mineral density

Decreased cardiorespiratory fitness

Loss of joint flexibility and mobility

Increase in body fat, particularly in the abdominal region

Changes in balance and motor coordination

Reduction in walking speed

Greater vulnerability to the development of chronic illnesses

These changes can translate into a reduced ability to perform daily tasks, such as climbing stairs, carrying shopping, walking for prolonged periods or getting up from a chair. However, age alone does not determine this decline. A healthy lifestyle, which includes a balanced diet, adequate sleep, stress management and the regular practice of physical exercise, can help to delay many of these changes.

How does physical exercise influence ageing?

Physical exercise acts on practically all the organs and systems of the body. During physical activity, muscles, bones, the heart and the brain are stimulated, triggering physiological adaptations that help to preserve function and delay some of the decline associated with ageing.

On the other hand, a sedentary lifestyle can accelerate some age-related changes. Remaining seated for long periods is associated with a loss of muscle mass, a reduction in cardiovascular capacity, an increase in insulin resistance and a progressive decline in autonomy.

It is now known that the body retains a significant capacity to adapt even in old age. Starting an exercise programme can therefore be beneficial at different stages of life and is one of the key measures for improving health, maintaining independence and promoting healthier ageing.

Benefits of physical exercise in ageing

Maintaining muscle mass and preventing sarcopenia

From the age of 30, muscle mass tends to decrease gradually. This process can accelerate after the age of 60 and, when associated with a significant loss of strength and physical performance, can contribute to the development of sarcopenia, a condition that compromises autonomy and is associated with a higher risk of falls, hospitalisation and dependency.

Strength training is currently considered one of the main strategies for preventing and countering the loss of muscle mass and strength associated with ageing. Exercises performed with free weights, machines, resistance bands or body weight stimulate muscle protein synthesis, contribute to the preservation of lean mass and improve muscle strength and power.

Scientific evidence demonstrates that older adults can significantly increase muscle strength and improve functional capacity, even when starting exercise programmes at an advanced age. These improvements can translate into greater ease in carrying out everyday tasks, such as getting up from a chair, climbing stairs or carrying shopping.

Strengthen bones and reduce the risk of osteoporosis

With ageing, bones progressively become more fragile due to the loss of bone mineral density. In women, this loss often accelerates after the menopause, whilst in men it generally occurs more gradually. Osteoporosis increases the risk of fractures, particularly of the hip, spine and wrist, which can significantly compromise mobility and independence.

Regular physical exercise plays an important role in maintaining bone health. Activities that involve moderate impact or mechanical load, such as walking, climbing stairs, dancing and strength training, stimulate bone remodelling and can help to preserve bone mineral density.

Although exercise does not eliminate the risk of osteoporosis, it constitutes one of the pillars of its prevention, when integrated into an approach that includes a balanced diet, an adequate intake of calcium and vitamin D, and other healthy lifestyle habits.

Improve cardiovascular health

Cardiovascular diseases continue to represent one of the leading causes of death worldwide. As we age, the heart and blood vessels undergo changes that can contribute to a reduction in cardiorespiratory fitness and an increased risk of high blood pressure, myocardial infarction and stroke.

Regular physical exercise improves cardiovascular system efficiency, contributes to blood pressure control, promotes a more favourable lipid profile, improves blood circulation and can reduce low-grade chronic inflammation.

Even moderate-intensity activities, such as brisk walking or recreational cycling, when practised regularly and suitably for an individual's physical condition, are associated with a reduction in cardiovascular risk and an improved quality of life.

Prevent and manage type 2 diabetes

Ageing is frequently associated with a decrease in insulin sensitivity, favouring an increase in blood glucose levels and the development of type 2 diabetes.

Physical activity improves glucose utilisation by the muscles, increases insulin sensitivity and contributes to better metabolic control. Simultaneously, it can help reduce visceral fat and facilitate the maintenance of a healthy body weight, which are relevant factors in the prevention of this disease.

When combined with a balanced diet and other health-promotion measures, regular exercise is one of the most effective non-pharmacological interventions for preventing and managing type 2 diabetes.

Maintain a healthy weight

As people age, there is frequently a decrease in basal metabolism and an increase in body fat, particularly in the abdominal region.

Exercise helps to counteract these changes by increasing daily energy expenditure and contributing to the preservation of muscle mass, which plays an important role in metabolism.

In addition to aspects related to body composition, reducing belly fat can help lower the risk of cardiovascular disease, insulin resistance and chronic inflammation, factors associated with less healthy ageing.

Protect brain function and reduce the risk of cognitive decline

The brain also benefits significantly from the regular practice of physical activity. During physical activity, cerebral blood flow increases and tissue oxygenation improves. Molecules that can promote neuron survival, brain plasticity and the formation of new connections between nerve cells are also released.

Several studies show that physically active people have a lower risk of developing cognitive decline and some forms of dementia. Although exercise is neither a cure nor a guarantee of preventing these diseases, it is associated with better preservation of functions such as memory, attention, information processing speed and executive functions.

Furthermore, maintaining good physical condition can help to preserve independence in everyday activities, including in people with mild cognitive impairment.

Improve mental health and emotional wellbeing

Ageing can bring significant emotional challenges, such as retirement, the loss of family members or friends, social isolation and the presence of chronic illness. These circumstances can increase the risk of anxiety, depression and a reduced quality of life.

Regular physical exercise is a relevant tool for promoting psychological wellbeing. During physical activity, endorphins and other neurotransmitters associated with the sensation of pleasure and wellbeing are released, contributing to the reduction of anxiety and depression symptoms.

The benefits are not limited to mood. Exercise can improve sleep quality, increase energy levels, boost self-esteem and promote a greater sense of autonomy. When done in group settings, it can also encourage socialisation, reduce social isolation and strengthen the feeling of belonging to the community.

Reduce the risk of falls

Falls represent one of the main causes of injury, hospitalisation and loss of independence in older people. They frequently result from the combination of several factors, such as decreased muscle strength, balance alterations, vision problems and the use of certain medications.

Exercise programmes that integrate strength training, balance, coordination and mobility have been shown to reduce the risk of falls. Activities such as Tai Chi, clinical Pilates, functional training or specific fall-prevention programmes can improve postural stability, increase confidence during walking and reduce the fear of falling, contributing to a more active and independent life.

Promoting independence and quality of life

The main goal of physical exercise in ageing is not just to increase life expectancy, but also to prolong the years lived with health, functionality and independence.

Good physical condition makes daily tasks easier, such as climbing stairs, walking longer distances, getting out of bed or a chair, carrying shopping, doing housework, playing with grandchildren, taking part in social activities and travelling.

Maintaining this functional capacity makes it possible to preserve autonomy for longer, reducing the need for help from others and contributing to a better quality of life.

Increase longevity

Regular physical exercise is associated with lower all-cause mortality. This benefit results from the combination of several positive effects, including better cardiovascular health, more effective metabolic control, lower chronic inflammation, preservation of muscle mass, reduced frailty and improved immune system function.

Although exercise does not prevent ageing, it can help ensure that it takes place in a healthier, more active and independent way.

How much exercise is recommended after the age of 65?

Current World Health Organization recommendations suggest that adults aged 65 and over should do weekly:

Between 150 and 300 minutes of moderate-intensity aerobic physical activity, or between 75 and 150 minutes of vigorous-intensity physical activity

Muscle-strengthening exercises involving all major muscle groups on at least two days a week

Functional balance and coordination exercises three or more days a week, particularly for those at increased risk of falls

Reduction of time spent sitting, breaking up prolonged periods of sedentary behaviour with short bursts of movement throughout the day

The exercise programme should be tailored to each person's capabilities, limitations and goals. When chronic conditions or functional limitations are present, it is recommended that its design and progression take into account individual assessment and, where appropriate, the supervision of qualified professionals.

Understanding ageing from an integrative perspective

Ageing is inevitable, but the way we age can be influenced by the choices we make daily.

Physical exercise is one of the most relevant interventions for preserving health, preventing chronic diseases, maintaining autonomy and promoting quality of life over the years. Its benefits extend far beyond physical condition, also encompassing cardiovascular, metabolic, bone, cognitive and emotional health.

It is never too late to start. Even small changes can yield significant gains when carried out consistently and suited to individual capabilities. Taking the first step can contribute to more years of life with independence, mobility and well-being.

Rita Xarepe | Physiotherapist and Clinical Pilates Instructor by APPI

Physiotherapist Card: 4209 | Order of Physiotherapists

Integrativa | Health and well-being as a lifestyle

This website and its content are for information purposes only and are not a substitute for medical or health professional advice.
